Euthanasia Consent Form - Lamar

I, the undersigned, do hereby certify that I am the owner or duly authorized agent for the owner of the animal described above.

I hereby order and give Bluebonnet Animal Hospital Circle C and its personnel the complete authority to euthanize the above described animal, and to dispose of its remains as specified below and in accordance with hospital policy. I hereby forever release the doctor and his/her agents from all liability for euthanizing or disposing of the above mentioned animal.

To the best of my knowledge and belief, this animal has not bitten any person in the previous 15 days and has not been exposed to rabies. I hereby grant permission for a postmortem study of my pet, if deemed necessary by the veterinarian. I request that my pet's remains be cared for as follows:
